Community Residences Summary
CR is a recognized provider in the field of mental health as a nonprofit service agency providing community based supports for adults and children with psychiatric disabilities. CR has provided services to individuals with disabilities for 35 years.
This is a clinical position providing comprehensive, recovery oriented mental health support and case management for adults with a wide range of psychiatric disorders, including co-occurring. The supports include assessments, connecting to benefits and entitlements, transitioning to permanent supported housing, and skills building to ensure success and tenure in long term housing. This position works independently under direct supervision performing a comprehensive array of services. The counselor performs assessments specific to the needs of a SMI/co-occurring population and maintains detailed electronic health records that meet state, local, and federal performance contract expectations.
Essential Clinical Responsibilities
Performs complex evaluations;
Maintains a diverse caseload of individuals with acute, chronic mental illness;
Develops and implements therapeutic treatment plans;
Collaborates with service professionals as participant as part of an Interdisciplinary Team (IDT);
Consults with professionals on a variety of problems including delivery of needed services for clinically complex cases;
Complete all documentation effectively, thoroughly, and in a timely manner. Maintain clinical and financial documentation in compliance with all relevant regulations and policies.
Education, Experience, Licensure:
Masters degree in Social Work, Counseling or Psychology preferred;
Bachelors degree in the human services field;
Preferable 2-years of experience working with individuals with chronic and persistent mental illness;
Licensed or licensed eligible, preferred.
