RN Coordinator/Advanced Practice Nurse WP
Provides the primary coordination of hospital-based hospice and palliative care; receives all hospice and palliative care referrals and arranges for admission to the hospice or palliative consultative service, interfaces with the patient/family, and with the attending physician; triages all incoming referrals and coordinates interdisciplinary team members required to best meet immediate needs of patient; initiates the care plan and discharge planning and communicates with the interdisciplinary team regarding patient goals and treatment plan; makes nursing assessments, judgments and treatment decisions based on customer needs and wishes and in accordance with level of professional skills.
Requirements:
  • Licensure from appropriate jurisdiction to practice nursing.
  • RN from NLN accredited nursing program. BSN preferred.
  • Minimum of two years of public health/community health, medical/surgical or hospice nursing experience.
  • Hospital based nursing a preference.
  • Strong communication and counseling skills of added benefit.
  • Certifications in Hospice/Palliative care a plus.
  • Nurse Practitioner or Advanced Practice Nurse strongly preferred.

Hospice of Prince George's County, our newest addition, joined the Capital Hospice family in August 2001. Also among the nation's most established hospices, HPGC has been serving Prince George's County since 1983. Because HPGC's hospice philosophy and approach to caregiving was so similar to that of Hospice of Suburban Maryland, the Board of Directors of each organization agreed that combining the hospices would result in a single, strong, Medicare-certified, non-profit program that will better serve the growing needs of the Prince George's County community.

Halquist Memorial Inpatient Hospice Center
When around-the-clock attention or respite care is needed, patients and families may turn to the Halquist Memorial Inpatient Hospice Center in Arlington, Va. Established in 1982, The Hospice Center virtually glows with warmth and many of the comforts of home. The only free-standing acute care inpatient facility in the area, the Hospice Center is accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Capital Hospice takes a life-affirming approach to hospice care, with an emphasis on patient comfort, dignity and choice. Hospice professionals assist patients and their loved ones in living each day to the fullest, in the most purposeful and meaningful way, consistent with their wishes and needs.

The interdisciplinary hospice care team includes a physician, nurse, social worker, chaplain, certified nursing assistants and specially trained volunteers. As part of the plan of care, we also offer physical, occupational, speech and other therapies as needed. Our art therapy program helps young people cope with terminal illness and loss. Hospice care professionals are accessible 7 days a week, 24 hours a day.

Capital Hospice's extensive bereavement program offers counseling, support groups, and workshops for individuals and families coping with loss. We also offer special weekend and day camps for children, adolescents, and adults who have lost a loved one.
