A list of Afghanistan war deaths included one U.S. Marine twice, once with an incorrect middle initial, age and home town. The item should have read: "Lance Cpl. Mark D. Juarez, 23, of San Antonio; 1st Battalion, 3rd Marine Regiment, 3rd Marine Division, 3rd Marine Expeditionary Force, based at Kaneohe Bay, Hawaii. Killed Jan. 9 in Helmand province."
